Ordering Telecommunications Services

Requests for Telecommunication Services must be submitted by your Agency appointed Telecom Coordinator.  If you do not know who your Telecom Coordinator is, please contact the Service Desk at 217-524-3648 (Springfield) or 312-814-3648 (Chicago).

Agency Telecom Coordinators must perform the following actions:

For all voice orders and requests for new lines, telephone equipment and accessories, LAN installations, moves, changes, fiber optics, and cellular services:

Effective August 1, 2019, any previous versions of the TSR will no longer be accepted. 

1. Complete: New Form E-TSR Form (v196B)
2. Email completed form to:DoIT.Prov@illinois.gov

1.    For Account Credit Requests: Proceed to and complete the Accounts Receivable Credit Request Form (ARCRF)
