IWIN Services

Requests for IWIN Services must be submitted by your Agency appointed IWIN Coordinator.  If you do not know who your IWIN Coordinator is, please contact the Service Desk at 217-524-3648 (Springfield) 312-814-3648 (Chicago).

Agency IWIN Coordinators must perform the following actions:

For all IWIN network connectivity requests:

1. Complete:WSR Page 1  WSR Page 2 CDMA  User Registry Information Form
   (New IWIN subscribers should also complete the LEADS Agreement Form  
2. Email completed forms to: DoIT.Prov@illinois.gov

For all other IWIN requests:

1. Proceed to and complete the appropriate IWIN Form
2. Mail completed form: DoIT.Prov@illinois.gov
