Freedom of Information (FOIA) Requests

This Freedom of Information Act ("FOIA") Request Form is provided to you as a courtesy. DoIT does not require that a FOIA request be submitted on a standard form. 5 ILCS 140/3(c). However, your use of this form helps us quickly process your request and provide a prompt response.

PLEASE READ BEFORE SUBMITTING: This DoIT FOIA online request form is ONLY for FOIA requests seeking DoIT public records. DoIT does not process FOIA requests for other State Agencies or local Illinois entities. If your FOIA request seeks public records that are not related to DoIT, please locate the appropriate State Agency. Also, DoIT FOIA does not answer general questions about DoIT. General questions can be emailed to DoIT.Helpdesk@Illinois.gov.
